How Medical Doctors Are Adding To Burden Of Fistula In Nigeria

Fistula is a health condition that most conscionable humans will not even wish for their enemies, especially when the condition is almost entirely preventable. Suffered by hundreds of thousands of women in Nigeria, VICTOR OKEKE in this report writes on the culpability of some surgeons in this case.

The experiences of 20-year old Deborah Joshua from Vandeikya Local Government Area of Benue State, Nigeria, to that of 22 year-old Linda Ojini, from Bamanga in Cameroon Republic, attest to the culpability of medical doctors in widening the fistula burden.

Deborah Joshua narrated her story, “After I got married, I became pregnant and when I was due for delivery I went to the hospital. I stayed at the hospital for three days- from Friday to Monday and I couldn’t deliver by myself. It was then that the doctor carried out an operation on me to bring out the baby. After that, the baby died and I also developed this sickness.

She has been experiencing urine leakage for over one year and six months now after she had the C-section.

She said that the doctor at the Katsina-Ala hospital where she had the C-section attributed the leakage to the long period she went through labour of child delivery.

Joshua has since been abandoned by the boy that got her pregnant but thanks to Fistula Care Plus, she has been healed.

Also, Alice Ebagu from Gakem in Bekwarra LGA, Cross River State, told the story of how she got married four years ago, but was not able to put to bed on my own during pregnancy, then opted for a C-section and then developed a fistula afterward.
“I was there with the problem. I did not know what to do but luckily one of my sisters called me that there is solution to this problem at the Ogoja Hospital. I thank God and that was how I came here,” she said.

She said the C-section was conducted at a private clinic she called Dr Otobo Clinic in Bekwarra.

Elizabeth Ebok Ayam, who is at the post-operation ward, also had a similar experience.

According to her, when she was pregnant, her mother took her to Dr Benson’s hospital in Ikom, Cross River State where the doctor told her that the baby had died in her womb.

“At the hospital, the doctor said that my baby had died inside the womb and he operated on me to bring out the baby, that was how the doctor spoiled me.

He came and knelt down and was begging my mother to forgive him as he as ‘spoilt’ her daughter. That is why I’m passing through all these.”
Also, 22 -year-old Linda Ojini, from Bamanga in Cameroon Republic, said she dropped out of an Arabic school from class six following an incident of unwanted pregnancy worsened by an escapee boyfriend. “I got pregnant and when my labour started I went to the hospital to deliver the baby but the baby could not come out and later when my baby came out, urine started coming out also.”

She explained that it was her brother who took her to a hospital in Bamanga, Cameroon where her mother is staying but was met with an unsuccessful treatment.

She was later referred to the Fistula centre in Ogoja by her uncle who she said, spent N4,500 to get to Nigeria.

Most women in the developing world, including Nigeria, are experiencing “unimaginable suffering” due to lack of effective care during pregnancy and childbirth.

According to medical experts, non-fatal maternal childbirth injuries such as Vesico Vaginal Fistula (VVF) often have devastating psychological and social consequences with an impact greater than maternal death.

VVF is an abnormal communication between the urinary bladder and the vagina that results in the continuous involuntary discharge of urine.

In other cases, it is called obstetric fistula, which is a hole between the vagina and the rectum or bladder that is caused by prolonged obstructed labour, leaving a woman incontinent of urine or faeces or both.

This condition has tremendous psycho-social and health consequences on the affected women as the uncontrolled urine or faeces leakage causing unpleasant odour results in social stigma and consequent neglect.

Most women affected are living in shame and isolation, often abandoned by their husbands and excluded by their families and communities.

According to the United Nations Population Fund (UNFPA), in Sub-Saharan Africa alone, between 3,000 and 130,000 of women giving birth develop fistula each year.

However, in up to 90 per cent of cases, the fistula can be surgically repaired.

Through the Fistula Care Plus project, the largest U.S. government-funded effort to date dedicated to treating and preventing fistula, EngenderHealth works to restore dignity to women with fistula and to prevent other women from developing the condition.

Engender Health Nigeria Country Project Manager; Chief Iyeme Efem, says Nigeria currently has over 200,000 women living with fistula with 12,000 new cases occurring every year.

The prevalence of fistula is much lower in places that discourage early marriage, encourage and provide general education for women, and grant women access to family planning and skilled medical teams to assist during childbirth.

Hence the burden of fistula rests largely on the women in poor rural communities of Nigeria and its severe nature puts a serious toll on the lives of those affected.

Majority of the victims are very young and without the basic elementary education. Most of them find it difficult to engage in any economic activity, surviving the hardship is very complicated and pathetic as coping is done in isolation and loneliness.

Some of the studies were explained in a presentation by Prof Oladosu Ojenbgede, during a ‘hands-on training on VVF repair workshop’ at the 51st annual scientific conference of the Society of Gynaecology and Obstetrician of Nigeria (SOGON) held at the Usmanu Danfodiyo University Teaching Hospital (UDUTH) Sokoto this week.

Also, last week, as part of its activities in Nigeria, EngenderHealth gathered a pull of several expert fistula surgeons in Ogoja, Cross River State, the location of one out of the 13 fistula hospitals the charity operates in Nigeria, to carry out surgeries on over 25 women suffering from fistula.
Most of the patients narrate how they developed the fistula following caesarean section operations to get the baby out after a prolonged obstructed labour and in all the instances, their babies died.

Dr Sa’ad Idris, Lead Fistula Surgeon, former Commissioner for Health, Zamfara State, explained how increased access to cesarean sections without regard to quality of care is causing an increase in Iatrogenic fistulas, a form of fistula unintentionally caused by a health care provider.

Unfortunately, Idris said that the trend of fistula is now changing in that most incidents are caused by lack of medical expertise during surgery. “We are seeing more fistulas caused by the medical personnel during the caesarean section- operation to remove the baby-maybe because of a lack of expertise or some other conditions which we are trying to see if we can liaise with the Society of Obstetricians and Gynaecologists (SOGON).”

He said Fistula Care Plus is sponsoring many doctors for specialised training on fistula treatment and to have round-table discussions so that the issue can be addressed right from the university.

“You don’t have to be a specialist to do a caesarean section but you have to be doctor. Now a doctor even at the House Officer level should be trained to know that if you are going to conduct a C-section you have to reflect the bladder very otherwise, what we are seeing more now is what we are seeing more is fistula following a C-section operation which is technically very difficult to do,” Idris added.

Iatrogenic fistulas are sometimes caused by inexperienced clinicians who are thrown into situations with many patients. According to data from Fistula Care Plus, of nearly 6,000 fistula repair cases in 11 countries, approximately 13 per cent were caused by surgical errors performed by all types of clinicians.

According to Chief Iyeme Efem, there is a big challenge of expanding the curriculum of medical training and education in Nigeria.

He said that a clinician is not trained and graduated from the medical school knowing how to do fistula surgery. “Even when you specialize as a gynaecologist, you still do not have the expertise to do fistula surgery. You need additional training. We look for all opportunities to talk to them.

During doctors’ meetings, we also have opportunities to talk to them telling them that their doctors are doing a lot of harm and causing a lot of trauma to pregnant women.”

“It is challenging that when these women go to hospital for delivery (and we preach delivery under trained healthcare provider and attending antenatal) when they do all of these, then have a C-section and end up with fistula. That is not right,” Efem said.

He noted that fistula is just one component of the morbidity and a marker on the poor health system including the skills of the surgeons. “We are now working on a strategy we call unlocking potentials, potentials that exist in certain levels -tertiary institutions, which can be unlocked to be able to do fistula at that level.”

Efem said that the minister of Health, Professor Isaac Adewole has given his approval for the 49 teaching hospitals in Nigeria to begin a programme of free treatment of fistulas in order to tackle the mounting backlog of women living with fistula in the country.

“The minister said that the skills are resident in the teaching hospitals and he has signed a memo to all the teaching hospitals waiving the fees for fistula repairs. He has waived fees for registration, and bed space and so on. We will support them by providing consumables,” Efem said.

Medical experts are of the view that healthcare authorities must ensure that with the rapid expansion of surgical care in low- and middle-income countries comes high quality care.

They said if rates of Iatrogenic fistula continue on the current trajectory, the caseload for fistula will remain steady, even if fistula from prolonged and obstructed labour no longer occurs.

Taking swift action will help guard against normalising iatrogenic fistula.

Doctors working with EngenderHealth under the Fistula Care Plus programmes called for the following five actions to address iatrogenic fistula: Standardise aetiology definitions, signs and symptoms used to classify fistula. This will allow doctors to analyse trends and make valid comparisons between facilities and settings.
They said increase awareness of Iatrogenic fistula in the obstetrician-gynaecologist and surgical community to accelerate improvements in surgical workforce training as well as clinical environment.

Secondly, effective improvements in surgical and post-operative team training and work environment- this applies to all clinicians, not just lower-level providers.
Thirdly, improve decision-making regarding surgical obstetrician gynaecology procedures. Preventing unnecessary surgeries is crucial to preventing iatrogenic fistula.
Fourthly, implement routine monitoring and reporting of iatrogenic fistula in settings where surgical ob-gyn care is provided.

Fifthly, preventing the need for surgery in the first place—by preventing unwanted pregnancy through family planning and preventing unnecessary caesarean sections and hysterectomy surgeries through proper antenatal care and skilled obstetric care—is critical to preventing iatrogenic fistula.
When surgery is needed, however, standards, guidelines and accountability mechanisms will help ensure high quality and optimal outcomes for mothers and babies.

How Early Child Development Can Tackle Out-of-school Challenge

WINIFRED OGBEBO writes that early child development can contribute meaningfully to nation building.

Early learning opportunities apart from other benefits, are key strategies to reducing out-of-school children challenge.
Early child development (ECD) has also been documented to be one of the most cost effective strategies for poverty alleviation.

At the opening of the 62nd National Council on Education (NCE) in Kano, recently, the permanent secretary, Federal Ministry of Education, Adamu Husseini, disclosed that 50 per cent of the cumulative 20 million out-of-school children in the world are in Nigeria.

He said the percentage represents 10.5 million of the 20 million out-of-school children in the world.

“Out of the country’s 10.5 million out-of-school children – the world’s highest number, sixty per cent of those children are in northern Nigeria.”

He attributed it to cultural factors, saying: “Almajiri-children, those of the nomadic pastoralists, boy and girl-child dropout, social miscreants, children living with disability, those of migrant fishermen and more recently, children displaced by insurgency, constitute the bulk of the affected children.”

Husseini added: “No nation can achieve economic prosperity without a sound, inclusive and functional education system. The security and stability of the country, to a large extent, depends on its ability to provide functional education to its citizens.”

For the benefit of hindsight, attempts in the past to provide free education whether at the federal or state levels has never been successful due to perhaps poor planning and implementation.

Nigeria has in the attempt to stabilize this wobbling sector introduced a number of programmes to harness basic education as existing ones failed to meet the educational needs and aspirations of the populace.

This led to the proposal of the universal basic education (UBE) by the Nigerian government.
Basic education includes early childhood education, primary, and lower secondary education.

This was welcomed by the populace who were unhappy that the 6-3-3-4 system which replaced the 6- 5-2-3 system and the earlier introduced universal primary education programmes has not fared better than the earlier introduced programmes.

The Universal Basic Education (UBE) Programme which bears 6-3-3-4 system code was introduced as a reform programme aimed at providing greater access to, and ensuring quality of basic education throughout country.

The UBE Programme objectives aimed at majorly ensuring an uninterrupted access to 9-year formal education by providing free and compulsory basic education for every child of school-going age under 6.

Intrinsically, the Universal Education Act of 2004 mandating every government in Nigeria to provide free, compulsory and universal basic education for every child of primary and junior secondary school age has not been followed.

The present economic hardship has even become a threat that has jerked up the already intolerable number of out-of-school children in the country.

The failure of government to adhere to the Universal Basic Education Act has undoubtedly given rise to the high cost of obtaining qualitative basic education.

To overcome the challenges of children roaming about the street and the challenges being encountered in the delivery of adult and youth literacy programmes, government has a big role to play.

The child friendly school concept, early child development (ECD) which UNICEF is advocating for, if comprehensively adopted by the various states in Nigeria, would help to tackle the issue of out-of- school challenge.
Speaking in Kano on a 2-day media dialogue on Early Childhood Development organised by the United Nations Children’s Fund (UNICEF) and the Child Rights Information Bureau (CRIB) of the Federal Ministry of Information, an Education Specialist with UNICEF, Swadchet Sankey Sankey, said the Lancet Medical Journal Survey (2016) showed that globally, 250 million children are at risk of not reaching their full potentials in education, health wise and overall development.

She said, “This is as across the world. Sixty million children under-five are also at risk of not making the best of impact in life, with Nigeria ranking among the ten countries with the largest number of children at risk of poor development.

Sankey listed UNICEF recommended policies to ensuring that Nigerian children fair well in early development and are protected from killer diseases to include ; two years of free pre-primary education, six months of paid maternity leave; and four weeks of paid paternity leave.

Nigeria currently has only one year of free pre-primary education and no paternity leave at all. Only about one in every 10 pre-primary children are enrolled in early education activities.

“The goal of any investment in early childhood development is for all children in Nigeria, whether poor or rich from conception through to school, attain their development potential,” she said.

However, the Education specialist explained that Nigeria still has a lot to do even though a lot has been done in terms of the health aspect of early childhood development vis-à-vis health, immunization, vitamins supplementation, prenatal care and post natal care.

She said it’s not just about the care of the child but also about the care of the mother

“A healthy mother who is pregnant will give birth to a healthy child and that translates to how well the child develops physically. Even the child’s brain development is affected by the biology, the gene, the environment and the health of the mother.

Sankey observed that though a lot has been done to ensure that prenatal and post-natal care is provided for women and children, there is still a lot that needs to be done.

“For example, ECD goes beyond immunization. When mothers go to the hospital for prenatal and postnatal care, what kind of capacity do they receive or the kind of stimulation activities can they do with children of different ages?

“ Simple things like eyes contact or giving the child feedback, singing and reading to the children- these are things that mother and fathers and all caregivers need to know how to do and why it is important to do these to the children even right from when they are still in the womb because children begin to hear even when they are inside.

“These are areas that we need to strengthen. I don’t think we are really doing well in all of these areas. We need to focus on how we can get communities and parents more into children development and in their children learning. Learning begins right from home. So even before the child begins to go to school; there is a lot that can be done to stimulate the child, to strengthen the brain and the child’s curiosity at home.”

The UNICEF Education Specialist said in terms of the learning aspect of early childhood development, there is a lot that needs to be done.

“We need to improve on the quality. It is not just enough to say that government is ready to fund pre-primary education, which is good. But research has shown that you need more than one year of pre-primary education for the child to be ready. But we have started. We need to ask of the quality of education within this one year that government is funding.

What is the capacity? Are teachers given better capacity? The school environment needs to be improved. If you go to the public primary schools, a whole school that is combining pre-primary and primary school, some of them don’t have toilets and when they have, the toilets are not enough. Children of that age should have their needs; specially dedicated toilet for them and they should have safe drinking water within the facility with soap and basins for them to wash their hands because at this age, they play a lot which is the natural way that children learn. So they need hygiene.
“The environment needs to be made cleaner and the kind of books that are provided for children in pre-primary school are very important. For this age, you need picture books and books that teach them simple words.

“We need to look at how we are training teachers in the colleges of education and the universities so that we are up-to-date. There is a lot of research and pedagogy around how early education should be done. We need to look at our curriculum and review that. We also need to build the capacity of our lecturers in the various institutions on this new thinking around their role in early childhood education, the skills the child needs to learn and what activities they need to do in order to address the particular skill?

“How do we use locally available resources and talking about toys for playing, it doesn’t have to be expensive toys. There are so many things around us that we can use in order to bring about these improvements.”

Sankey also canvassed for improved funding because, according to her, pre-primary education is a subsector that needs to be paid attention to.
“It is not an addendum rather it is part of the basic education system. We should stop treating it as though it is an addendum. It is important because a research done in Jamaica showed that children who had quality pre-primary education for at least more than one year when they become adults, earned 25 per cent more than those who did not.

Meanwhile, the assistant director, Child Right Information Bureau, Federal Ministry of Information and Culture, Olumide Osanyinpeju, said there is need for all to rise up for the propagation, as early childhood development can make the difference between life and death.

“It depends on nurturing care that ensures health, nutrition, responsive caregiving, safety and security, accessible and quality early learning such as pre-primary education.
He said the emotional, social and physical development of young children has a direct effect on their overall development and on the adult they will become.

“ That is why understanding the need to invest in very young children is so important, so as to maximize their future well-being.”

Highlighting the importance of ECD, UNICEF’s Education Specialist, said, “We need to get our early learning and pre-school education right.

“ Investments in ECD can improve education outcomes, promote equity, build a skilled workforce and yield high returns and benefit society.”

Manchester City captain Vincent Kompany says the Premier League frontrunners have taken a leaf out of their arch rival’s book and will never stop believing they can win games even in the dying seconds.

City have maintained an eight-point lead at the top of the Premier League with last-gasp 2-1 victories over Huddersfield and Southampton in their past two games, both late winners scored by Raheem Sterling.

The Southampton goal came six minutes into stoppage time and sparked jubilant scenes at the Etihad Stadium on Wednesday.

Digging out maximum points in such circumstances was a hallmark of many of Manchester United’s title successes under Alex Ferguson and City captain Kompany recognises it is a good trait to acquire.

“It’s too early in the season to talk about titles but it is just good to know that you can have that feeling that at any time of the game you can make something happen. I felt it could still happen even that late in added time,” said Kompany.

“United used to do it to everybody else, to be honest.

“But it’s a long season. We have done it so far but, if at the end of the season we have done it consistently, I will be happy about it.”

City have won their past 12 league games but the tightness of the past two matches, after some emphatic wins earlier in the run, possibly indicates a change in approach from opponents when facing City.

“From the beginning of the season through to now the games have changed completely.

“I wouldn’t say it is something to complain about because it just gives you that knowledge that you are going to be on top of the team you are playing but it is something that we have to learn to deal with.

“The sooner we find a way to unlock these teams, the better it will be for our season.”

Despite the exhilaration of Sterling’s late strike against Saints, Kompany hopes City can return to winning games more convincingly.

He said: “I wouldn’t bank on it happening every week, so the sooner we start scoring earlier the better.”

Sterling has now scored 13 goals this term, already the most prolific season of his career.

Kompany said of his team-mate’s improvement: “I think it’s maybe an inner hunger, maybe that desire to be among the best. I can only encourage him to carry on on this path. I’ve seen the work he has put in behind the scenes and that is the way forward.”

Mr. President Welcome To Kano, Your Political Stronghold

The long-awaited official visit of the President and Commander-in-Chief of the Armed Forces of the Federal Republic of Nigeria to his most solid political base in the whole country, i.e. Kano State will become a reality anytime from now.

It is expected that Mr. President will embark on a 3-day official visit to the most populous State in the federation during which he will commission some landmark projects executed by the administration of His Excellency, the Executed Governor of Kano State, Dr. Abdullahi Umar Ganduje OFR, inspect on-going ones, and also some private agro-based industries and meet with the cream of the society made of the Ulama, traditional rulers, businessmen and political bigwigs and lay the foundation stone of other gargantuan ones.

For long, Kanawas have been longing for this historic visit since the epoch-making elections of 2015 when the APC swept away the then ruling PDP at federal and state levels. But due to some intervening variables, Mr. President could not make this visit prompting speculations by some political funambulists to insinuate that the most populous politician in Kano State has forsaken his political base which is very far from the truth. Kano people will forever remain loyal and solidly supportive of Mr. President and I am sure he will never jettison Kanawas through thick or thin.

Equally worth-mentioning is that the amiable Governor of Kano State, Dr. Abdullahi Umar Ganduje, OFR is also a Buhari loyalists and he has demonstrated this in both words and actions on several occasions which has angered some desperate politicians who accuse him of disloyalty, “anti-party” activities and betrayal of their “cause and ideology” as if he is subservient to someone rather than being the chief executive of Kano State who is accountable to the Almighty and the people who voted for him en masse. Mr. President, be rest-assured that the Governor, his administration and majority of the people of the State are yours and when you visit, you will see huge crowds of Kanawas that will line the streets to welcome you chanting in chorus Maraba! Maraba! Maraba! Sai Buhari! Sai Buhari! Sai Buhari! Sa Mai Gaskiya! Sai Mai Gaskiya! Sai Mai Gaskiya! Sai Baba! Sai Baba! Sai Baba!

Though not a fortune-teller, I am sure the successful visit of Mr. President to Kano State will give some desperate politicians sleepless nights because they will then realize that they have no any political base upon which to realize their self-centred political ambition because Kano ta Buhari ce da Ganduje.

Mr. President, let me be the first to welcome you to Kano State because during this visit you will see the laudable progress so far recorded by the present administration of His Excellency, Dr. Abdullahi Umar Ganduje supported by loyal lieutenants. In fact, in the last two years since the Ganduje administration came to power, we have seen unprecedented transformation in the form of infrastructural development, implementation of people-centred policies and programmes especially empowerment of youth and women as well as the determined efforts of the State Government to ensure the security of lives and property of the citizenry.

These are not a mean achievement especially as some assume that the governor is not the right stuff to assume the governorship of the State and they have not relented in their futile efforts to cast him in bad light. However, the people are much wiser and things are becoming much clearer by the day.

It is really a great achievement for the Ganduje administration to consistently pay salary to public servants and still execute these numerous money-guzzling projects within this period of two years since its assumption of office. While other States are grappling with salary liabilities for many months, the State Government has been paying a colossal monthly wage bill of about N10 billion. Mr. President, this is a great achievement and the Governor deserves a pat on the back for this has demonstrated his prudent management of scarce financial resources, acumen, transparency and accountability.

Mr. President is scheduled to commission two mega hospitals inherited and completed by the Ganduje administration which will impact directly on the lives of the people unlike some elephant projects executed by others for mere prestige and self-deification. The government also installed state-of-the-art equipment with a view to providing efficient and effective healthcare services to the teeming people of the State. The completion of these two projects in particular has clearly demonstrated that Dr. Ganduje is not a short-sighted politician as others wilfully refuse to complete projects initiated by their predecessors however important they are to the betterment of the lives of the people that defied rains and sun to vote them into office.

Apart from spending billions of Naira to complete the two health facilities, the Ganduje administration also expended the sums of N964,883,036.56 and N1,999,053,126.54 for the procurement of modern equipment and furniture for the 250-bed Zoo Road Paediatric and 250-bed Giginyu Specialist Hospitals respectively, these equipments and furniture have been successfully installed ready for commissioning by our august visitor in sha Allah. During his recent visit to the hospitals, the Hon. Minister of Health was highly impressed by these ultra-modern pieces of equipment which he described as the first of their kind in the country that will go a long way in preventing medical tourism by some Nigerians that flock to foreign countries in search of quality healthcare services.

Other gigantic projects to be commissioned by Mr. President include the multi-billion Naira Madobi-Panshekara underpass embarked upon by the present administration in order to ensure safe motoring, bring to an end chaotic traffic jam and beautify the environment, the CBN Quarters Road, among a host of many others all executed within a space of two years of the present administration.

Also, Mr. President will embark on inspection visits to some projects which include the resuscitated Kano Agricultural Supply Company (KASCO) that has been provided with a second modern fertilizer blending line and the company now supplies assorted fertilizer to even the federal and some state governments. Unlike before, KASCO is now a very profitable entity ably run by a new team of visionary and dedicated management staff which is to the credit of the Ganduje administration.

There is no doubt that the Buhari administration has clearly made its policy on agriculture known loud and clear so that the age-long over-reliance on oil will be a thing of the past and the Ganduje administration has unwaveringly towed this line. That is why it has been according the agricultural sector much-needed support in terms of provision of fertilizer and other inputs and credit support to local farmers.

 

To this end, the President will inspect some private rice mills established by some businessmen to support the federal government initiatives on local rice production to reduce its importation that guzzles billions of US Dollars annually.

One project that is dear to Gov. Ganduje is the on-going multi-billion Naira ultra-modern skills acquisition centre project aimed at training the youth of both sexes in about 22 vocational skills in an effort to make them self-reliant and productive. It is this gigantic project that Mr. President will inspect especially as contract for the supply and installation of equipment worth about N2 billion at the centre has been awarded recently by the State Government.

Another inherited project which the Governor is bent on completing under a PPP arrangement is the Kanawa Modern Market (Kano Economic City) and that is why Mr. President will lay the foundation of this ambitious project that will transform the commercial landscape of the State and even the West African sub-region. Upon completion, the market will consist of thousands of shops of various sizes, hotel, and other modern facilities/amenities. In addition, Mr. President will visit the famous Kantin Kwari Textile Market to witness the on-going modernization projects embarked upon by the State Government in line with the consistent policy of the Ganduje administration to modernize all major markets in the State aimed at creating a conducive commercial/business environment.

So, Mr. President, Kano State is working as it is undergoing rapid socio-economic transformation under Gov. Ganduje. Remarkable achievements have been recorded and the Kanawas are very appreciative of this progress which you will see in sha Allah when you visit this ancient and historic city of Kano, how i wish you will stay for months, to be able to visit all the projects embarked upon by Ganduje, as two days will certainly not be enough to go round and see them all. All this despite the recent economic recession in the country that affected all tiers of government, but our amiable and far-sighted Governor has weathered the storm. Apart from initiating new projects, the Governor has vowed on several occasions to complete all inherited projects that have direct bearing on the lives of the teeming people of the State and we have seen that he has kept to his promises as a gentleman with vast experiences garnered from various public spheres.

Ikpe claimed that the Office of the National Security Adviser in May 2014 paid Azibaola the sum of $40m from the $1.2bn approved by Jonathan.

An EFCC investigator has claimed that ex-President Goodluck Jonathan's cousin,  Robert Azibaola, got $40m from the $1.2billion arms deal fraud.

The investigator, David Ikpe, who stands as the 11th prosecution witness of the Economic and Financial Crimes Commission, made this known at the Federal High Court in Abuja on Thursday, November 30, 2017.

Ikpe claimed that the Office of the National Security Adviser in May 2014 paid Azibaola the sum of $40m from the $1.2bn approved by the ex-President.

Ikpe said, “In March 2014, the then NSA, Sambo Dasuki, wrote a memo to the President titled, ‘Intervention on urgent national security project’ in which he sought the approval of Mr. President for $1bn to handle a number of sub-heads including unmanned area or pipeline monitoring installations.

“The NSA made an additional request for $250m in April 2014.

“The President gave approval to his request.

 

“The President approved the release of $1.2bn to the NSA for the purpose it was sought.

“In May 2014, the sum of $1.2bn was then released from the NNPC account under the title ‘intervention on urgent national security project.’”

Continuing, the investigator admitted that Azibaola was not the only one paid from the $1.2bn released by the NNPC saying “It was this fund that was disbursed to the various recipients.

“I confirmed that the $40m was part of the $1.2bn released by the President for urgent national security intervention project.”

When asked if he interviewed Dasuki, being the man at the centre or obtained his statement in the course of the investigation, Ikpe said he did not.

However, he said that the EFCC did not set out to investigate the $40m transaction saying the transaction was discovered on investigation of other allegations contained in the petition sent to the EFCC by the new leadership of the ONSA in 2015.

The defence lawyer, reportedly showed Ikpe exhibits of Dasuki’s authorisation of the sum of $40m to Azibaola.

Meanwhile, the witness admitted on Tuesday that he and his team members obtained during a search of Azibaola’s office, documents detailing how the defendants spent the money released to them by the NSA office.

He said it must be the documents of the original case at the EFCC office when the defence lawyer him asked to produce them during the Thursday’s proceedings.

The anti-graft agency alleged that the former president authorised the Nigerian National Petroleum Corporation to release the sum of $1.2bn to the ONSA for “urgent national security project.”

Azibaola, his wife, Stella, and their firm, One Plus Holdings Nigeria Limited, are being prosecuted by the EFCC on seven counts of money laundering before the court.

The EFCC alleged in the seven counts that the three defendants unlawfully received $40m from the then NSA, Sambo Dasuki, allegations which they denied.

The Vice President, Federal Republic of Nigeria, Professor Yemi Osinbajo on Thursday reaffirmed the Federal Government’s commitment to eradicating poverty, hunger, unemployment and other related vices in the country.

The Vice President while launching the Nationwide Small and Medium Enterprise Clinic (MSME) and the Government Enterprise Empowerment Programme (GEEP) in Osogbo stated that‎ 20,000 people would benefit from the initiative in Osun.

The MSME is a Federal Government’s Empowerment Initiative under the office of the Vice President initiated to support Small and Medium Enterprises in the country.

Osinbajo stated that the scheme was initiated by President Muhammadu Buhari in a bid to support small businesses and enterprises by providing the people soft interest free loans to run their businesses.

He said the present administration led by President Muhammadu Buhari having seen the SME as the powerful economic drive decided to take the bull by the horn to empower the sector as part of efforts to stimulate and re-energise the economy.

Osinbajo expressed confidence in the administration of President Buhari, saying “I have confidence in the fact that our President will take Nigeria and Nigerians to greater heights.

According to him, we are here in Osun to launch another milestone achievement of the present administration in the country in fulfillment of our electoral’s promises to the people of Nigeria.

“The Nationwide Micro, Small and Medium Enterprise (MSME) and Government Enterprise Empowerment Programme (GEEP)‎ are enterprise intervention programmes of Mr. President to empower the small businesses in the country.

“This scheme was initiated to provide interest free loans to those who are into small businesses so as to empower them and as well stimulate our economy.

“To us, this scheme remains the engine of the economy and as a serious government, we have keyed into it to ensure that our people are empowered and taken out of the abject poverty and hunger..”We have made arrangement to empower 50, 000 people ‎in Osun under this scheme so as to further strengthen the people’s commitment to businesses and as well encourage those who are into SME.

“We are bent on our commitment to continue to deliver on our electoral promises as our government is determined to judiciously utilize the nation’s ‎resources for the betterment of our people”, Osnbajo added.

While commending the people of Osun for being enterprising and productive, the Vice President said, “I have seen in the people of this state the zeal to economic productivity and prosperity”.

“My visit to this state has made me to see how resourceful the people of this state are. I have seen that Osun is one of the engine of agro-allied economy in Nigeria as this could be easily identified in the zeal of our people to economic growth and development.‎

He lauded Governor Rauf Aregbesola for bringing to his people the much needed dividends of democracy through his numerous human and capital infrastructural developments.

“My visit has made me to see some of the good works of this administration particularly on roads, schools, security among others”, Osinbajo emphasised.

Speaking earlier, Governor of Osun, Ogbeni Rauf Aregbesola charged whoever is ready to work and earn a living to key into the laudable programme.

He said the programme will empower about 20,000 beneficiaries in total starting with 10,000 beneficiaries who will get a none interest loan of at least 50,000 each.

According to Aregbesola, “for those of us who are ready to work and earn money, this is an opportunity for you. I advise you to key into the this programme and do something good for yourself.

“We are happy about this because it will go a long way in alleviating the sufferings of people and assist in our efforts to banish hunger and poverty.

Maureen has been married for eight years without a child and now her husband, Godwin, has gotten her maid pregnant and wants her out of his house.

"Dear Pulse,

My name is Maureen, a 36-year-old woman. I am very bitter and disturbed at the moment as my husband of eight years has thrown me out of our house because of my maid whom he got pregnant.

In the years I have been married, I have not been blessed with a fruit of the womb and this has made me very unhappy. Before now, my husband, Godwin, had been very supportive and had always stood by me until his family started putting pressure on us.

It began when his mother came to the house last year and made it very clear that the family had given me an ultimatum to give them a child or they would kick me out.

That day, I cried so much and asked God why He would allow me to suffer so much shame and indignity in the hands of my husband's family.

It is not as if we had not tried everything humanly possible for us to get a child. I had gone to so many hospitals, had countless fertility tests, went to men of God and even native doctors all in a bid to get pregnant.

But all of these proved abortive. I have never taken in nor had a miscarriage all my life but the results of the tests showed that I was neither barren nor anything wrong with me. Even the men of God kept assuring me that I would have my own baby at the right time.

But now, all the hopes I have been given has been dashed after Godwin threw me out of the house after he got our maid pregnant.

I did not know that he had been sleeping with our 20-year-old maid till she got pregnant and during interrogation, she confessed that he was the one reponsible.

I was mad when she told me it was my husband that got her pregnant. When I asked Godwin, he was not even sober about it and said that it is now a proof that he is not the problem and that if I would not want to stay inthe same house with my maid as his second wife, I was free to leave.

But last month, his mother and sisters came and with the support of my husband, threw me out of the house.

I have been staying with my sister since then and all efforts to get my husband to take me back has proved abortive as he has declared that as long as I do not want my maid to be my co-wife, I must never come back to his house.

Please, I need someone to help me on what to do here.

I advised Malami to meet with Maina – DSS DG, Daura

Appearing before the House of Representatives panel investigating the former pensions boss, Abdulrasheed Maina on Thursday, the Director-General of the Department of State Services (DSS), Lawal Daura, says he advised the Minister of Justice and Attorney-General of the Federation, Abubakar Malami, to meet with Mr Maina.

Highlights:

  • He gave the advice after Mr Malami sought his opinion on whether or not to hold the meeting.

“The attorney general of the federation sometime in 2016 or 2015, because it was not through a formal document, placed a call to me when he was outside the country,” he said.

“He wanted me to advise him whether to agree to see Maina or not in that foreign country.

“I responded to the attorney general that he should accept to see Maina but he should not see him alone. He should see him with a third party which he did.”

  • Daura also said the DSS provided security for Mr Maina’s after the former pension boss sent a letter, claiming that his life is in danger.

He added: “The letter was scrutinised and fears expressed by Maina were also to a large extent established.

“Because of the right to life which is guaranteed by our constitution universally recognised, Maina’s case was directed to be looked into so as not to allow his life to be in jeopardy.”

  • He said no agency has written to the DSS seeking to arrest Maina, and added that the secret police had nothing to do with his reinstatement.

Rap Mogul Russell Simmons Steps Down After Sex Accusations

Russell Simmons, who helped bring hip-hop into the mainstream with Def Jam Recordings, said Thursday he was quitting his business empire after fresh allegations of sexual abuse.

The 60-year-old — who unlike many in the rap world had until recently an uncontroversial public image — disputed the latest claim against him but said he viewed the mounting public spotlight on sexual misconduct to be “a time of great transition.”

“The voices of the voiceless, those who have been hurt or shamed, deserve and need to be heard,” Simmons said in a statement.

“As the corridors of power inevitably make way for a new generation, I don’t want to be a distraction so I am removing myself from the businesses that I founded,” he said.

The announcement came after “Rachel Getting Married” screenwriter Jenny Lumet said that Simmons assaulted her in 1991 when she was 24 and he was a decade older.

Writing in The Hollywood Reporter, Lumet said Simmons forced her into his New York apartment, with his driver refusing her pleas to be taken to her own home, and had sex with her even though she said no.

In his statement, Simmons said he remembered the evening differently but acknowledged that “her feelings of fear and intimidation are real.”

“While I have never been violent, I have been thoughtless and insensitive in some of my relationships over many decades and I sincerely and humbly apologize,” he said.

Simmons earlier denied accusations by model Keri Claussen Khalighi that he assaulted her in 1991 when she was 17.

Simmons and fellow producer Rick Rubin founded Def Jam in the mid-1980s as hip-hop became a growing force, signing acts such as the Beastie Boys, LL Cool J and later Public Enemy who brought rap to a wide US audience for the first time.

Simmons also promoted Run-D.M.C., the trailblazing rap trio which included his brother Joseph.

While stepping back from music production, Simmons has remained active in entertainment promotion with his Rush Communications firm.

He also runs a fashion brand and a company of yoga apparel and studios.

Simmons, himself a yoga enthusiast, said that a “new and diverse generation” of executives would take over his companies and that he would turn a yoga studio into a “not-for-profit center of learning and healing.”

He has also been active in charities, notably supporting animal rights and advocating veganism.

Nasarawa Government Has Put in Place Modalities To Strengthen NSHIP

The Nasarawa State Government, said modalities have been put in place towards mainstreaming the Nigeria States Health Investment Project (NSHIP) currently being piloted in the State.

Governor Umaru Tanko Al-Makura, made this known when he received at the World Bank team on mid-term review assessment exercise, at Government House, Lafia.

The Governor disclosed that he has penultimate week issued an executive order aimed at exploring measures towards seeking legal frame work and through legislation to domesticate the NSHIP project into the mainstream of the State health centre development agency.

A press statement signed by the Chief Press Secretary to the Governor Musa Elayo and made available the press, Al-Makura assured World Bank team that when such measures are put in place, it would ensure the sustenance of the project even when his tenure elapsed.

He also reiterated his administration commitment towards expanding the scope of NSHIP projects in thestate, through deepening collaboration in the area of manpower development in order to surmount the deficit in the over 700 primary healthcare centres.

According to the statement, the Governor however expressed his administration resolve to seek the approval of relevant agencies towards increasing the number of intakes in the schools of nursing and health technology to improve service delivery.

Earlier, the World Bank team leader, Dr. Benjamin Loevinson, told the governor that they were a mid-term impact review assessment of NSHIP projects, in which Nasarawa is one of the pilots State.

According to him, their visits to Primary Health Care facilities has revealed that Nasarawa has recorded tremendous improvement in services provided in PHC’s and has graduated from D grade to B grade since the project took off in the State, some years back.

A fresh graduate of the Michael Okpara University of Agriculture in Abia state has excitedly shared pictures of how he worked as a bricklayer to survive through University.

Orji Joseph, who graduated with second class honour in Mathematics was happy to share the pictures of himself carrying bricks with the caption that was apparently addressed to his friends saying,  “This is why I miss class and probably dozed in class”

 

Orji said that he survived working as a bricklayer when he was in 300 and 400 level.

He wrote:

I planned on sharing this photo @ the peak of my career… Nevertheless, I believe it’s out here today to encourage someone to keep trusting God.

Laugh small & celebrate with *Mouwa* This was how I survived 300l & 400l. This was one of the reasons I made a Good Chief Usher on campus. From this came the fund for writing & presenting my project. For the photocopies & all; from this came the fund  #To_my_Course_mates; this is why I missed classes & probably dozed in class(lol).

Tech: Google is bringing its own take on Instagram-like stories to YouTube (GOOG)

The concept is a little different from that of Instagram, Facebook, and WhatsApp stories, though.

YouTube is adopting the "stories" format made popular by Instagram and Snapchat, but with its own spin.

The new feature is called "Reels," and will allow creators to take advantage of a dedicated tab inside their channel's page to put stacks of 30-second clips there to share with their audience.

Creators can shoot video from within the app, and then do some basic editing like adding text, filters, stickers, or some background music.

Reels won't be displayed at the top of the app, so they won't have the same prominence as stories have on Facebook's services and Snapchat, but YouTube told TechCrunch that it might reconsider their position if the feature takes off.

There's also another major difference: Videos inside Reels won't disappear after 24 hours, as stories normally do, and won't need to be under a centralised feed either.

This means that youtubers will be able to create multiple, topic-based Reels, and fill them up at will for people to enjoy at any time — putting the focus of Reels on the content's story rather than the person's.

YouTube said that the feature will first arrive in beta, but didn't specify a date, say how long it plans for it to stay in beta, or which creators will get to use Reels first.

Allardyce arrives at Everton for Rooney's Hammer-blow hat-trick

Sam Allardyce saw Wayne Rooney cap his first Premier League hat-trick in six years with a wonder strike as Everton beat fellow strugglers West Ham United 4-0 on Wednesday after the Merseysiders announced he was finalising terms to become their new manager.

Allardyce’s imminent appointment will end Everton’s long hunt for a successor to Dutch boss Ronald Koeman, who was sacked last month.

Veteran English manager Allardyce arrived at Goodison Park in the company of Everton owner Farhad Moshiri and the pair saw Rooney, in his second spell with his hometown club, score twice inside the opening half-hour.

But it was his 66th-minute effort that will be talked about for years to come.

From fully 60 yards out, Rooney struck the ball over the head of his former England team-mate Joe Hart after the Hammers goalkeeper had rushed out of his box to boot the ball into Everton’s half.

Victory moved Everton up into 13th place in the table and go left them five points clear of the relegation zone.

West Ham, managed by former Everton boss David Moyes, remained in the bottom three and two points shy of safety.

Everton went ahead in the 18th minute after Hart brought down Dominic Calvert-Lewin to concede a penalty.

Hart saved Rooney’s spot-kick only for the experienced forward to head home the rebound.

And 10 minutes later Everton were 2-0 up when Rooney, given his league debut by Moyes as a 16-year-old in 2002, shot home from 12 yards out following a cross from Tom Davies.

At 2-0 down the Hammers still had a chance to force their way back into the match, but Everton goalkeeper Jordan Pickford saved a penalty from Manuel Lanzini after Ashley Williams hacked down Diafra Sakho.

Then came Rooney’s extraordinary hat-trick goal before Williams headed in Gylfi Sigurdsson’s corner 12 minutes from time.

If this was indeed David Unsworth’s last a match as Everton caretaker boss, it was a fine way for the club’s former Under-23s manager to bow out.

Unsworth had been in temporary charge since Koeman was sacked on October 23 and he had made clear his desire to take on the job permanently.

But Wednesday’s victory was just Everton’s second in eight matches under Unsworth, during which time they’ve lost five and conceded 20 goals.

‘Strong leader’
An Everton statement released earlier Wednesday announced Allardyce’s upcoming appointment.

“Sam Allardyce will be at USM Finch Farm (Everton’s training ground) this afternoon to finalise terms on becoming the new manager of Everton Football Club,” it read.

The Daily Mail reported Allardyce had signed an 18-month deal and quoted Moshiri saying: “We need a strong leader. He gives us that.”

Allardyce, 63, had ruled himself out of the running earlier this month, saying he did not feel Everton were convinced he was the right man.

But after the Liverpool club’s pursuit of Watford head coach Marco Silva proved fruitless, Everton returned to Allardyce.

This will be the 10th English club managerial position taken up by Allardyce, who established his reputation at Bolton Wanderers and guided Crystal Palace to safety last season.

When he left Selhurst Park in May, Allardyce said he didn’t want another management job, but the chance to turn around Everton’s fortunes appears to have lured him back.

Everton invested heavily following last season’s impressive seventh-place finish, but results have fallen well below pre-season expectations.

Allardyce has previously led Palace and Sunderland to safety from precarious situations.

His achievements at the Stadium of Light earned him his dream job with England, but that turned sour after only one match, with Allardyce stepping down in September 2016 following a newspaper sting.
