The FDA just approved an opioid that's 1,000 times stronger than morphine

Many people have expressed concern that this drug could make the opioid crisisworse, wondering “do we really need this on the market? As with most medical advances, there are pros and cons.

The FDA approval of Dsuvia means there’s now another opioid on the market-an extremely powerful one. It’s 1,000 times more potent than morphine and 10 times more potent than fentanyl.

Many people have expressed concern that this drug could make the opioid crisisworse, wondering “do we really need this on the market? As with most medical advances, there are pros and cons.

The drug is a sublingual (under the tongue) version of sufentanyl, which was first made in 1974 and is given intravenously. On the benefit side, Dsuvia has a unique ability to provide high-level pain relief in a short time, without need for IV access. That means that trauma victims can get relief quickly, so one use for Dsuvia will be for medics who are literally on the battlefield; the Pentagon strongly supported approval of this drug. It may also be useful in the operating room to provide sedation and anesthesia, especially in people for whom it is difficult to get an intravenous line in, such as some people with cancer.

What are the downsides? Well, a drug so powerful that can be taken by mouth has the potential for abuse. Synthetic opioids, such as fentanyl, provide a “high for many. However, just because the drug is a thousand-fold more potent doesn’t mean the “high is a thousand times stronger: it just means that the dose that’s needed to have the effect is much less.

The FDA has taken some common-sense steps to reduce the likelihood of abuse. The drug is going to be supplied in a single-use disposable container. It will not be available for home use or through retail pharmacies; only in “medically supervised settings, such as hospitals or the military. The FDA has also mandated that Dsuvia will be “reserved for use in patients for whom alternative pain treatment options have not been tolerated, or are not expected to be tolerated, where existing treatment options have not provided adequate analgesia, or where these alternatives are not expected to provide adequate analgesia. It will also contain a “black box warning, the highest warning the FDA issues to indicate there are serious hazards associated with the drug. None of these guarantee that the drug will not be abused, people being as they are, but it reduces the likelihood of misuse and diversion.

While the US continues to have a serious opioid problem, I don’t believe that the approval of Dsuvia is likely to significantly escalate it, given the precautions around it. While its uses in the ER and OR are marginal, it’s not a thousand or even ten times more dangerous than fentanyl, which is being synthesized and sold illicitly on the street. It does have unique benefits in certain situations that are worth adding to the medical toolkit, and in my opinion, those outweigh the risks in this case.

The Corporation disclosed this in its monthly Financial and Operational report released in Abuja on Wednesday.

It said that between August 2017 and August 2018, the federation and joint ventures (JV) received the sum of N879.02billion and N651.4billion respectively.

The NNPC explained that the Federation Crude Oil and Gas Revenue, Federation Crude Oil and Gas lifting, were classified into Equity Export and Domestic crude.

It explained that this crude were lifted and marketed by corporation and the proceeds remitted into the Federation Account.

It noted that Equity Export receipts, after adjusting for Joint Venture Cash Calls, were paid directly into the Federation Account domiciled in Central Bank of Nigeria (CBN).

The corporation explained that domestic crude oil of 445,000 bpd was allocated for refining to meet domestic products supply, and payments were effected to the Federation Account by NNPC.

This, it said was done after adjusting crude and product losses and pipeline repairs and management costs incurred during the period.

On the crude oil and gas export sales, the report noted that sales for the month of August stood at 470 million dollars.

According to the report, the sales indicate an upsurge of about 78million dollars in relation to July oil and gas export figures of 391.91million dollars.

It further indicated that crude oil export sales contributed 337.62million dollars which represented 71.83 per cent of the dollar transactions compared with 283.43million dollars contribution in the previous month.

“Export gas sales during the period amounted to 132.38million dollars.

“The August 2017 to August 2018 crude oil and gas transactions involved crude oil and gas export worth 5.26billion dollars, it said.

The report explained that based on the above sales figures, a total export receipt of 450.24million dollars was recorded in August 2018 as receipt against 382.65million dollars in July 2018.

“Contribution from crude oil during the period, amounted to 336.43 million dollars, while gas and miscellaneous receipt stood at 101.33million dollars and 12.48million dollars respectively, the report noted.

A further breakdown of the figures showed that out of the export receipts, 142.31million dollars was remitted to the Federation Account.

The sum of 307.93million dollars was remitted to fund the JV cost recovery for the month of August, 2018 to guarantee current and future production.

“Total export crude oil and gas receipt for the period August 2017 to August 2018 stood at 5.23billion dollars out of which 3.74 billion dollars was transferred to JV Cash Call as first line charge and the balance of 1.49 billion dollars paid into the Federation Account, it added.

Power outage hits Lagos airport, temporarily disrupts operations

Murtala Muhammed International Airport

Flight operations were on Wednesday evening disrupted at the Murtala Muhammed International Airport, Lagos following a power outage.

The News Agency of Nigeria (NAN) reports that the outage occurred at about 6.05pm and lasted for over an hour before it was restored.

The situation placed the nation’s busiest airport in total darkness, leaving airline officials, passengers and other airport users frustrated as they could not proceed with the boarding processes.

An official of the Federal Airports Authority of Nigeria (FAAN), told NAN on condition of anonymity that the outage was caused by disruptions from the Ayobo and Ikeja Transmission Lines which supplies power to the facility.

“What happened was that there was intermittent disruption of power supply and it was unstable during the period it lasted but it was eventually restored, the source said.

The General Manager, Corporate Affairs, FAAN, Mrs Herrientta Yakubu, who confirmed the development, said power had been restored to the airport.

An aviation expert, Capt. John Ojikutu, decried the ugly development, saying as a Category 1 airport, the MMIA ought to have independent power supply, especially in case of emergencies.

“FAAN is expected to have independent power line from Ikeja and Ayobo transmission lines as planned for MMIA in 2007; what has happened to that proposal?

“This is a shame on all of us including the Nigerian Civil Aviation Authority (NCAA).

“Again, how did NCAA certify the MMA if the power problem that could affect the airport safety facilities has not been resolved?

We have a long way to go with the NCAA safety oversight functions, he said.

How Too Much Screen Time Can Affect Child Development – Greensprings

Screen time has become an inescapable reality of modern childhood, with children of every age spending long hours in front of screens, from televisions to computers, tablets, and smartphones etc. Although spending time in front of screens can sometimes be for positive use, as children are able to access various educational apps that help stimulate their brains. Nonetheless, screen time has a long-term negative impact on children.

According to Dolapo Fatoki, Head of School at Greensprings School Ikoyi Campus; we understand that busy working mums sometimes leverage on technology to get a break from their children, by keeping them occupied with games on their phones or tablet; but we strongly encourage parents to explore more creative ways to engage their children with physical activities. These will help prevent them from having a sedentary lifestyle in future and ultimately lead to better health; she added.

The period between birth and age three, in medical terms is called the critical period because the changes that happen in the brain during these tender years become the permanent foundation upon which all later brain function is built.

The human brain within this period develops very quickly and is particularly sensitive to the environment around it.

Some of the negative impacts of too much screen time on children based on research are enumerated below:

Children who consistently spend more than 4 hours per day watching TV are more likely to be overweight.

Kids who view violent acts on TV are more likely to show aggressive behaviour, and to fear that the world is scary and that something bad will happen to them.

Teens that play violent video games and apps are more likely to be aggressive.

It is so important for parents to keep tabs on their children’s screen time and set limits to ensure they are not spending too much time in front of a screen.

When asked what steps parents can employ in order to curtail their children’s screen time, Mrs. Okrikri, the Secondary School Principal noted that it may be difficult to totally stop children from spending time on screens in this digital era, however, there must be some form of parental control on these devices and more importantly adult supervision.

A number of studies have emerged that connect delayed cognitive development in children with extended exposure to electronic media.

According to developmental psychopathology expert, Prof Lynne Murray, of the University of Reading, “there are well-established literatures showing the adverse effects of prolonged screen time on the cognitive development of children under three years of age. This has therefore led to the recommendation by the US Pediatric Association that there should be no screen time for children before the age of three.

NLNG’s $7bn Train-7 Expansion Project Excites Local Vendors

Local vendors in the Niger Delta region have expressed interest in the USD$7billion gas production expansion project of the Nigeria Liquefied Natural Gas Limited christened NLNG Train-7 in Bonny Island, Rivers State.

To this end, the vendors, under the aegis of Association of Niger Delta Vendors in Oil and Gas, would be participating in a proposed business roundtable on the project slated for November 22 and 23 for young entrepreneurs in the oil and gas sector in Bonny Kingdom.

Already, the local organising committee had commenced an awareness interactive session as part of preparations for the summit, and an online portal had been established for bookings and enquiries.

Chris Jumbo, the organising committee chairman for the event, said the workshop was strategic in several ways as it would also address some of the associated challenges ahead of the take-off of the NLNG Train-7 project.

He stated that more than 100 indigenous vendors and many business groups had signed up for the workshop.

He noted that engaging vendors from the locality would encourage the people to support the expansion project and explore inherent business opportunities while ensuring a safe environment for business.

Jumbo said“ We want to applaud the organisers of this novel initiative. It encourages community participation, and when that is factored in, the environment becomes conducive.

“Leadership of various professional groups in Bonny Kingdom are favourably disposed to the idea of this gathering to examine the potential of the proposed NLNG Train-7.

“We are, therefore, commending the Minister of State for Petroleum Resources, Executive Secretary of Nigerian Content Development and Monitoring Board (NCDMB) and the Special Adviser to the President on Niger Delta Matters, Prof. Charles Dokubo, for putting together this workshop.

“This is the first time a host community is being recognised by including them while carrying out the public awareness for opportunities for prospective vendors.

He said he believed that issues to be discussed at the workshop would help to mitigate any industrial disputes before they occur due to the proactive measures taken by the management of NLNG.

“The commitment, passion and content brought to the table are quite impressive. We wish to encourage these collective efforts by the relevant stakeholders to use the project to drive development of the Niger Delta region,”Jumbo added.

Unai Emery Better Than Arsene Wenger, says Shkodran Mustafi

Arsenal

Shkodran Mustafi says manager Unai Emery has brought a level of tactical detail to Arsenal that was not present under predecessor Arsene Wenger.

Wenger stepped down as Arsenal boss after 22 years in May, having endured growing criticism and supporter hostility for his perceived limitations as a tactician and a coach, as well as for the Gunners’ underwhelming transfer record.

Emery has overcome a difficult start to lead Arsenal on a 14-match unbeaten run in all competitions, presiding over some thrilling football in the process, and Mustafi admitted he particularly appreciates his new manager’s meticulous style.
“For me personally the biggest thing was that he is someone who really knows what he wants from each player,â€ Mustafi said of Emery’s impact in a press conference to preview Arsenal’s clash with Portuguese giants Sporting in the Europa League. “He tells you exactly what he wants you to do, but at the same time he gives you freedom.

“When we don’t have the ball he tells you exactly where to stand and how to get the ball from the opponent… it is a good mix between freedom but still very disciplined and having the same plan in the whole team, and that is what we saw especially against Liverpool.

“I think we are very good attacking and that makes the defensive work easier. We are pressing higher up the pitch and that is making it easier to defend. And while we are attacking we are trying to prepare to be in the right positions.
Arsenal lie fifth in the Premier League after 11 matches, six points behind leaders Manchester City and just one point off a top-four place. They are also progressing well in the Europa League, having won all of their opening three matches to surge clear of Sporting at the top of Group E.
One more victory will be enough to all-but secure their place in the knockout stage, but Emery insisted that his team will not underestimate Sporting. The Portuguese giants have also made a positive start to the new campaign under Jose Peseiro, who replaced Sinisa Mihajlovic when the Serb was sensationally sacked just nine days after being appointed in June.
“They have very good players, Emery said. “They changed the coach but it’s a very big match for them in their thinking about getting the first position [in the group].

“We will go into the game with big respect for them.

Arsenal will be without Nacho Monreal (hamstring), who has returned to training with the squad but will not be risked against Sporting. Summer signing Konstantinos Mavropanos also remains sidelined with a groin problem, while there is still no return date set for Laurent Koscielny as the club captain continues his recovery from a torn Achilles sustained in May.
